JOB SUMMARY: To plan, organize and conduct Speech Therapy program in hospital, to facilitate development and rehabilitation of mentally, physically or emotionally handicapped.
Minimum Job Qualifications:
Education: Graduate from an accredited school of Speech Therapy.
Experience: Previous experience preferred.
License/Certifications:
Licensed by the North Carolina Board of Speech Therapy.
BLS required.
ESSENTIAL FUNCTIONS:
Evaluates individual’s functional capacity, as ordered, in regards to activities of daily living, home management and vocational/role performance, using assessments including ROM measurement, manual muscle testing, sensory testing, motor performance evaluation, functional positioning assessment, cognitive/ perceptual assessment, and functional performance evaluations for activities of daily living, home management and vocation.
Plans individualized treatment program suited to each individual’s physical capacity, intellectual level, and personal goals to upgrade the individual to maximum independent with self-care and home management to prepare individual for return to employment, to assist in restoration of function, to prevent further disability and to aid in adjustment to disability.
Maintains quality and up-to-date clinical records, including evaluations, re-evaluations, and progress reports, plan of treatments, physician orders, discharge summaries and screens.
Implement treatment plan through selection of appropriate tasks, exercises, or modalities that will assist individual develop or regain skills necessary for meeting their goals and maximizing their functional independence.
Evaluates and documents progress appropriately and adjusts treatment accordingly.
Consults with other Rehabilitation professionals so that treatment will be integrated and coordinated to allow patient to receive the benefit of an organized multi-disciplinary approach.
Designs and constructs adaptive equipment for individual necessary for maximizing their functional independence in their work/living environment.
Performs on-site assessments of home and work environment to determine appropriateness for maximizing individual’s functional independence with daily performance requirements as appropriate.
Requisitions supplies and tools.
For use in patient care at the hospital.
Sets up patient with necessary equipment to perform task and cleans equipment according to infection control policy and procedure.
Fills out billing sheet/documentation describing service given and routes it to appropriate billing station.
Performs other duties as assigned.
Must be willing to receive all required vaccinations (i.
e.
, flu shot, etc.
).
